The One Day TCP (Transitional Care Planning) Follow-on Course is designed for younger family members and / or friends who have already attended an Introductory F&F Course and have completed the SPECAL Care Profiling work on behalf of their person with dementia (rather than the spouses / partners or older friends of the person).
Duration: One day (09.15 – 17.00)
Numbers: Maximum of six families per course
You will learn about:
- The care home selection process
- The timing of any future move
- How to ensure that the care home becomes SPECAL sensitive
- The key elements of the initial 72 hour intensive care approach led by the family
- how to prepare and present a SPECAL Passport
- The art of visiting
- How to protect and sustain the carer’s relationship with their person
